Russians Say Progress Vehicle Brought Down by FOD

Roscosmos_logoMOSCOW (Roscosmos PR) –Roscosmos emergency committee have reviewed investigation results of the contingency with Soyuz-U and cargo Progress MS-04 December launch from the Baikonur Space Center.

The cause of the accident was off-nominal mechanical separation of the launch vehicle’s third stage and the cargo spacecraft. The members of the emergency committee established the following:

  • The most likely cause of the contigency was the third stage liquid oxygen tank opening as a result of exposure of 11D55 engine destruction elements that occurred in result of fire and further destruction of the oxidizer compound pump.
  • The cause of the oxidizer compound pump’s fire could be possible in case of foreign particles entry into the pump cavity or possible violation 11D55 engine assembly technology.

The plan of priority actions to ensure the next Progress MS-05 secure launch will be submitted in the near future.

    The infamous “foreign particle” strikes again. No wonder we’ve beaten them to the Moon.

    P.S. I probably should elaborate. They have a very long history of reports closed with a “foreign particle”. But the engine-maker typically knows that something is up and e.g. for S5.85 there were revisions to the turbopump bearings to address the “foreign particle” blow-ups. Every time the State Commission closes out with a “foreign particle”, it come to mean “we have no clue what went wrong and were able to narrow it down to a subsystem but not the specific fault scenario”.

  • I think you’re misinterpreting the Russian definition of FOD. The “Foreign Objects” in question were placed there by Foreign Agents looking to undermine the progress of the Great State Commission. This is covert sabotage without a doubt!

    Otherwise it would have been DOD – Domestic Object Debris/Damage. Stalin tried for years to root out that problem and made remarkable progress. THAT problem can be solved by simply rounding up the guilty parties and sending them to the GULAG!
